This Parental Consent Form allows your child to participate in the general activities of the CCBC Summer Kids Camp 2024 carried out on the premises of our church. Summer Kids Camp uses a variety of different activities and approaches in fulfilling its mission and purpose. These activities include, but are not limited to, (Bible studies, sports, discipleship classes, discussion groups, outdoor activities, relay and various games, mission projects, group activities, etc.) As the parent or legal guardian, I the undersigned, certify that my child, named in this Parental Consent Form, has my express permission to participate in all activities, of any nature, carried on within the premises of CCBC. If I desire to limit my child’s participation in any event, I understand I must submit my wishes in writing to Summer Kids Camp organizers. I understand that I will be notified in the case of a medical emergency involving my child. However, in the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event my child is injured or becomes ill.
